    -e 1) What happened

SystemInfo.deviceUniqueIdentifier generates different device id's depending if wireless is enabled or not. I guess this happens due to fact that some hardware does not declare their mac addresses if wireless is disabled and your engines choses to use android_id or something else. (See mechanics of mac address from http://android-developers.blogspot.com.tr/2011/03/identifying-app-installations.html, and a comment from unity expert here: http://forum.unity3d.com/threads/unity-4-2-systeminfo-deviceuniqueidentifier-regression.194047/)

The devices we have seen these problems include Galaxy Tab 3; Kindle devices

    2) How can we reproduce it using the example you attached.

There are already screenshots depending on wireless enabled or disabled.

    To reproduce:

Be sure to use a problematic device such as Galaxy Tab 3.
For example, screenshots belong to Kindle Fire HD (3rd Generation)

    a) Be sure that application has not executed yet.
    b) Enable wireless (if not enabled)
    c) Open application and note the device id
    d) Kill the application
    e) Turn off the wireless
    f) Open the application and see the difference

    Good luck!
